Alain suffered from severe bone loss. There remained very little cancellous bone. The good quality bone for implantation was the dense cortical bone structure, qualified as bone type D1 (Dense Bone | SNOMED CT 36173007) and D2 (Trabecular Bone | SNOMED CT 122557009).

Dr Genchev in total fixed 20 basal dental implants and kept 2 conventional implants that were stable. Dr Genchev selected Ihde Dental BCS and TPG Uno implants.

Upper jaw basal dental implants
On the upper jaw, Dr Genchev fixed 15 basal dental implants. In the posterior part of the maxillary he placed on each side two tubero-pterygoid implants Ihde Dental TPG Uno behind each sinus. They were fixed into the pterygoid process of sphenoid bone (Classification: SNOMED CT 72891007).
Anteriorly, in position 16 and 26 he placed on each side Ihde Dental BCS that were fixed into the pyramidal process of the palatine bone (Classification: SNOMED CT 84154003).
In the anterior part of the upper jaw Dr Genchev placed 9 dental implants Ihde Dental TPG Uno and Ihde Dental BCS that were anchored into the nasal floor cortex. It is the hard cortical bone that creates stability for the dental bridge.
Lower jaw basal dental implants
On the lower jaw, Dr Genchev removed 2 conventional implants that were not stable and kept 2 stable dental implants from which he removed the crowns.
He extracted the devitalized teeth that were not stable. He fixed 5 new basal dental implants. The number of implants depends on the quality of the bone.
Where the bone was completely atrophied at the back on both sides Dr Genchev used very thin, short implants to accommodate the lack of bone. Dr Genchev selected Ihde Dental BCS to use the cortical bone for maximum stability. In position 26 and 37, He placed Ihde Dental BCS that were anchored into the Mylohyoid line.
